Re: JSLS 2005;9:463–465 Heated, Humidified CO2 Gas Is Unsatisfactory for Awake Laparoscopy

چکیده
Appropriate conscious sedation is a critical component for successful awake laparoscopy. It is not surprising that the patients in both cases experienced pain during laparoscopy, since only 50 g of fentanyl citrate was administered in the first case, and 100 g in the second case. The protocol used for conscious sedation must be of sufficient efficacy to allow the awake procedure to continue with little or no patient discomfort. I would recommend my protocol for conscious sedation with the adjunct of heated and humidified CO2 for the successful performance of awake laparoscopy. The fact that Dr Crabtree’s second patient experienced pain following insufflation with only 200 mL of CO2 suggests that his methodology needs to be refined.

Heated, Humidified CO2 Gas Is Unsatisfactory for Awake Laparoscopy
BACKGROUND The necessity for general anesthesia represents an impediment to using a laparoscopic approach for some procedures that are otherwise performed with the patient under local anesthesia using a conventional open technique. Heating and humidifying the insufflation gas reportedly reduces perioperative pain associated with a CO2 pneumoperitoneum, thus enabling awake laparoscopy. METHODS...
متن کاملEffect of heated and humidified carbon dioxide on patients after laparoscopic procedures: a meta-analysis.
OBJECTIVE To systematically analyze the role of heated humidified carbon dioxide (CO2) in laparoscopy. METHODS Clinical trials on laparoscopic procedures using standard dry CO2 versus heated humidified CO2 for pneumoperitoneum were analyzed. RESULTS Ten randomized controlled trials on 565 patients were analyzed. In both the fixed and random effect models, postoperative pain was significantl...
متن کاملThe Effects of Insufflation Conditions on Rat Mesothelium
Aim. The aim of this investigation was to examine the alterations in the peritoneum after cold dry CO2, heated dry CO2, and humidified heated CO2 at pressures equivalent to intraperitoneal pressures used in human laparoscopy. Methods. Eighteen rats were divided into 4 treatment groups-group 1: untreated control; group 2: insufflation with cold dry CO2; group 3: insufflation with heated, dry CO2...
متن کاملImproved Outcomes for Lap-Banding Using the Insuflow® Device Compared with Heated-Only Gas
BACKGROUND AND OBJECTIVES Preconditioning gas by humidification and warming the pneumoperitoneum improves laparoscopic outcomes. This prevents peritoneal desiccation and detrimental events related to traditional cold-dry gas. Few comparisons have been done comparing traditional cold-dry, heated-only, and humidified-warmed carbon dioxide. METHODS A prospective, controlled, randomized, double-b...
متن کاملInsufflation with Humidified and Heated Carbon Dioxide in Short-Term Laparoscopy: A Double-Blinded Randomized Controlled Trial
BACKGROUND We tested the hypothesis that warm-humidified carbon dioxide (CO2) insufflation would reduce postoperative pain and morphine requirement compared to cold-dry CO2 insufflation. METHODS A double-blinded, randomized, controlled trial was conducted to compare warm, humidified CO2 and cold-dry CO2. Patients with benign uterine diseases were randomized to either treatment (n = 48) or con...
